CARE COACH TO VISIT WAUCONDA TOWNSHIP OFFICES

Monday, April 25th, Lake Forest Hospital’s Care Coach will be stationed at the Wauconda Township parking lot from 10:00 – 1:00 p.m.  The Coach is staffed with health care professionals providing free health prevention programs to all members of the Wauconda community.  The services provided include the following health screenings: blood pressure, blood sugar, cholesterol, bone density, depression and lung function.  The Coach also provides immunizations for children and adults, referral and advocacy services, and health and wellness counseling.  For further information call (847) 535-6709.
